Synopsis[]
After a woman (Maika Monroe) moves with her husband (Karl Glusman) to Bucharest, isolated by the language barrier, she begins to suspect that a man watching her from the apartment building across the street might be a serial killer who has been beheading women throughout the city.

- Julia goes to see Charade (1963) at the cinema. This is the same movie that Jay (also played by Maika Monroe) goes to see with Greg at the beginning of It Follows (2014).
- The original screenplay depicted the events as being set in Brooklyn, New York.
- The serial killer "The Spider" was simply known as "Headhunter" in the script.
- The original script by Zack Ford was featured on Hollywood's 2016 Black List, an annual list of unproduced most-liked spec scripts. Ford wrote the script under a pseudonym "Carl Young".
- In a 2022 interview, Chloe Okuno spoke of the contrast in styles between her previous work on V/H/S/94 (2021) and Watcher (2022), as well as influences: "I also love Roman Polanski's Apartment Trilogy (Repulsion (1965), Rosemary's Baby (1968), The Tenant (1976))... The other movies I think on Watcher that I was referencing were Perfect Blue (1997) and Lost in Translation (2003), Se7en (1995). And for the most part, those are movies that really take their time. Also I can really appreciate something where it is in a little bit more of a classic style and it's a slow burn and it's about building a visual language and pacing it in a way that's tied to the character, tied to Julia. It is a little bit slower and more deliberate. But they are two obviously extremely different modes of horror, but I really loved working in both of them."
- Maika Monroe also plays another character named Julia in the movie Tau (2018).
