- Jack Frost 2: Revenge of the Mutant Killer Snowman (2000) [Bloody Girl]: Impaled through the mouth when the snowman (voiced by Scott MacDonald) launches an icicle at her. Her body is shown again later on pinned to the column when Christopher Allport and David Allen Brooks go searching for the snowman.
- Justice League: The Flashpoint Paradox (2013) [Wonder Woman]: Voicing 2 versions of the character: Flashpoint Wonder Woman Perishes by the doomsday wave along with Cary Elwes. The Real Wonder Woman lives.
TV Deaths[]
- Sym-Bionic Titan: The Phantom Ninja (2010) [Xeexi]: Stomped on by Ilana (Tara Strong), Lance (Kevin Thomas), and Octus (Brian Posehn) while in Titan form. She manages to launch a probe to her master General Modula (Don Leslie) before dying.
- Young Justice: Humanity (2011; animated) [Black Canary/Red Inferno]: Red Inferno is melted into slag (alongside Red Torpedo (Jeff Bennett)) after falling into molten lava while fighting Red Volcano (also voiced by Bennett). Black Canary survives the episode.
- Avengers: Earth's Mightiest Heroes: Who Do You Trust (2012; animated) [Skrull Madame Hydra]: Dies offscreen and reverts back to her true form.
- Star Wars: Rebels: Path of the Jedi (2014; animated) [Hera Syndulla]: Killed off-screen along with Steven Jay Blum), and Tiya Sircar by Jason Isaacs) Vanessa, Steven, and Tiya's deaths were only visions by Taylor Gray. Vanessa survives the episode in reality.
- Star Wars: Rebels: The Protector of Concord Dawn (2016; animated) [Phoenix Four]: Shot down by the protectors.
- Star Wars: Rebels: Double Agent Droid (2017; animated) [Hera Syndulla/PZ-7/Female Intelligence Officer]: Voicing 3 characters: "Female Intelligence Officer" is killed in an explosion by an overload from Chopper created by Hera. Hera and PZ-7 survive the episode.
- Young Justice - Outsiders: Princes All (2019; animated) [Black Canary/Queen Ilona DeLamb-Markov]: Queen Ilona is stabbed to death (off-screen), alongside her husband (Bruce Greenwood), by a speedster assassin. Black Canary survives the episode.

- Metal Gear Solid 2: Sons of Liberty (2001) [Olga Gurlukovich]: Shot in the head by John Cygan in an act of sacrifice for Quinton Flynn and her daughter.
- Star Wars Jedi Knight II: Jedi Outcast (2002) [Jan Ors]: Is kidnapped by Desann (Mark Klastorin) and his apprentice Tavion Axmis (Kath Soucie). In a battle with Kyle Katern (Jeff Bennett), Desann threatens to kill Jan and we hear the sound of a lightsaber and Jan screaming not long. However when Kyle does battle with Tavion, she reveals Jan is alive and it was a trick so that Desann could follow him into the Valley of the Jedi.
- Narc (2005) [Daniels]: Shot to death by Michael Madsen (off-screen); we only see Michael fire his gun.
- Mass Effect 2 (2010) [Ereba/Nef]: "Nef" died of a brain aneurysm after having sex with Natalia Cigliuti, some time before Maggie Baird's loyalty mission; "Ereba" survives the game.
- Mass Effect 3 (2012) [Captain Lee Riley/Ereba/Officer Jordan Noles]: "Captain Riley" sacrifices her life to protect her men from Reaper forces if the player doesn't send one of their squadmates to help them; all three characters can also die in the Refusal ending, when the Reapers wipe out all sentient life in the galaxy.
- Metal Gear Solid V: The Phantom Pain (2015) [Dr. Strangelove]: Dies of asphyxiation after Christopher Randolph seals her inside an AI pod.
