Ghost in the Shell 2: Innocence (2009; anime) [Kim]: Brain fried while being used to interface with the Locus Solus factory by Togusa (Crispin Freeman).
Mass Effect: Paragon Lost (2012; anime) [Captain Toni]: Perishes when the Collector ship he is on crash-lands into the human colony Fehl Prime.
Batman: Bad Blood (2016; animated) [The Heretic]: Shot in the chest by Talia (Morena Baccarin).
Code Geass: Lelouch of the Re;surrection (2019; anime) [Zuee]: Ordered by the newly resurrected Lelouch (Johnny Yong Bosch) through Geass to kill himself, he blows himself up just as C.C. (Kate Higgins) jumps out of the way.
Superman: Red Son (2020; animated) [Superior Man and Guy Gardner (voice)]: Voicing a duel role, "Superior Man" disintegrates after "Lex Luthor" (Diedrich Bader) overloads him with excess energy at the end of a fight "Superman" (Jason Isaacs), who looks on in horror. "Guy Gardner" presumably drowns after being blasted into the ocean by "Superman" as he tries to stop him.

Code Geass: Lelouch of the Rebellion: The Collapsing Stage (2008; anime) [Andreas Darlton]: Blasted to death by Lelouch (Johnny Yong Bosch) with the Gawain's Hadron Cannons after stabbing the Gloucester piloted by Cornelia (Mary Elizabeth McGlynn) while under Geass.
Bleach: The Espada Gather! Aizen's Royal Assembly (2009; anime) [Aisslinger Wernarr]: Crushed to death by falling debris after being defeated by Uryu (Derek Stephen Prince).
Blassreiter: Drifting Hearts (2010; anime) [Hermann Saltza]: Shot by Beatrice (Lydia Mackay) when she tries to shoot an energy attack towards Malek (Micah Solusod). His body disappears while dying with Malek and Amanda (Jamie Marchi) by his side, and appears as a spirit for the last two episodes.
Bleach: Kira, The Battle Within Despair (2011; anime) [Charlotte Chuhlhourne/Findorr Calius]: Chuhlhourne dies after having all his life energy drained by Yumichika's (Brian Beacock) Zanpakuto; he is brought back to life as a by Mayuri (Terrence Stone) in Bleach: Thousand-Year Blood War in the episode Marching Out the Zombies; Findorr survives the episode.
Bleach: Hisagi's Shikai! The Name is... (2011; anime) [Findorr Calius]: Stabbed through the head by Shuhei (Steve Staley) with his Zanpakuto.
Bleach: Changing History, Unchanging Heart (2014; anime) [Kūgo Ginjō]: Cut down in combat by Ichigo (Johnny Yong Bosch), dying on the ground soon afterwards.
One Piece: Looking for the Answer! Fire Fist Ace Dies on the Battlefield! (2016; anime) [Portgas D. Ace]: Dies from his wound after shielding Luffy (Colleen Clinkenbeard) from a magma-imbued punch by Akainu (Andrew Love); after asking Luffy to pass his final words to his loved ones, he dies smiling.
Marvel's Spider-Man: Sandman (2017; animated) [Flint Marko/Sandman]: Crushed into a sandy pulp by his daughter (Sofia Carson) after failing to reason with her, thus effectively killing him.

Transformers: Dark of the Moon (2011) [Stratosphere]: Possibly died when Steven Jay Blum detonated the remaining Mechtech inside him as he went down.
Metal Gear Rising: Revengeance (2013) [Andrey Dolzaev]: Blows himself up atop an oil plant core in front of Quinton Flynn.
Far Cry 4 (2014) [Paul "De Pleur" Harmon]: Can be killed by James A. Woods if the player wishes to do so.
inFAMOUS Second Son (2014) [Reggie Rowe]: Falls to his death after letting go of his hand from Troy Baker due to being covered with concrete by Christine Dunford.
Battlefield Hardline (2015) [Carl Stoddard]: Shot several times in the chest by Nicholas Gonzalez.
Mad Max (2015) [Scabrous Scrotus]: Dies from head trauma when Bren Foster forcefully removes the chainsaw blade from his head.
Halo 5: Guardians (2015) [SPARTAN-II Frederic-104 & Sangheli Jul M'dama]: Travis's Sangheli character Jul is stabbed in the neck by Mike Colter before being pushed into a crevice in the ending cutscene of the game's first mission. Frederic survives the rest of the game.
Star Wars Jedi: Fallen Order (2019) [Jaro Tapal]: Shot multiple times by several Clone Troopers (Dee Bradley Baker), after distracting them away from pursuing Cameron Monaghan. He later reappears as a Force Ghost to him.
The Last of Us Part II (2020) [Large Rattler]: Shot by Ashley Johnson after he reveals the location of Laura Bailey.
