Impostor (2001) [Nelson Gittes]: Accidentally shot by Gary Sinise during a shootout with guards, he dies as Gary kneels by his side

The Equalizer: Breakpoint (1986) [Terrorist Leader]: Shot to death, along with the other terrorists, by Edward Woodward.
Monsters: Leavings (1991) [Officer Mancini]: Shot to death (off-camera), along with John Christopher Jones, by the Frankenstein-like cop (John Robert Bloom); we only see the cop firing as the episode ends.
Gargoyles: Grief (1995; animated) [Emir]: Presumably dies of unspecified circumstances after he sealed off the complex beneath the sphinx after he became the avatar form of Anubis (voiced by Tony Jay).
Monk: Mr. Monk is Someone Else (2009) [Adrian Monk/Frankie DePalma]: Playing a dual role, "Frankie" is hit by a bus while crossing the street. "Adrian" survives the episode.

Fallout 2 (1998) [Aradesh]: Died of unknown causes at some point after the events of the last game; his death is confirmed when talking to his daughter (Tress MacNeille).
