- Star Trek: The Motion Picture (1979) [Klingon First Officer]: Vaporized, along with the rest of the Klingon crew, when their ship is "absorbed" by V'Ger. (Tom was wearing heavy Klingon make-up for the role).
- Star Trek III: The Search for Spock (1984) [Klingon Crew Member]: Killed in an explosion, along with Stephen Liska and the other Klingons, when they board the Enterprise, unaware that William Shatner has activated the ship's self destruct sequence. (Tom was wearing heavy alien make-up for the role).
- Friday the 13th Part V: A New Beginning (1985) [Jason Imposter]: Impaled on a tractor harrow when he gets knocked out of a barn window by John Shepherd (after John also knocks off the mask revealing it to be Dick Wieand). Note: due to the dual portrayal of this role, it'll be listed for both actors.

- Star Trek: The Next Generation: Descent, Part I (1993) [Borg Drone]: Neck broken when he is grabbed by the throat by Brent Spiner and thrown against a wall. His death is seen again in a simulation, later in the episode, when Brent recreates the incident on the holodeck. (Tom was wearing heavy alien make-up for the role).
- Star Trek: Deep Space Nine: The Sword of Kahless (1993) [Soto]: Stabbed in the back with a bat'leth by John Colicos when he attacks Terry Farrell. (Tom was wearing heavy alien make-up for the role).
- Star Trek: Deep Space Nine: Empok Nor (1997) [Cardassian Soldier]: Shot with a phaser by Andrew Robinson. (Tom was wearing heavy Cardassian make-up for the role).
- Star Trek: Enterprise: Impulse (2003) [Seleya Crew Member]: Killed in an explosion, along with the rest of the infected Vulcan crew, when the Enterprise crew are forced to overload the Seleya's power grid to escape. (Tom was wearing heavy alien make-up for the role).
- Star Trek: Enterprise: Carpenter Street (2003) [Xindi-Reptilian Guard]: Shot with a phaser by Scott Bakula. (Tom was wearing heavy alien make-up for the role).
- Star Trek: Enterprise: Borderland (2004) [Klingon Soldier]: Neck broken during a fight with Alec Newman and Fernando Chien when they take over the Klingon ship. (Tom was wearing heavy Klingon make-up for the role).
As a professional stunt performer, Tom has probably "died" in place of many other actors.
