The Land Before Time VI: The Secret of Saurus Rock (1998) [Littlefoot]: Fallen to death in dream. he survive the film.
From Within (2008) [Aidan]: Shot in the chest by Kelly Blatz.
My Sister's Keeper (2009) [Taylor Ambrose]: Dies (off-screen) of leukemia with Sofia Vassilieva by his side while having sex after prom. (Nudity Alert: censored in the dark as we don't see anything below his chest.)
Kaboom (2010) [Smith]: Killed (off-camera) in a car crash (along with all the other major characters) when they go over a collapsed bridge at the end of a chase. We only hear the crash after seeing close-ups of each person reacting in horror, ending on a freeze-frame of Thomas.
A Nightmare on Elm Street (2010) [Jesse Braun]: Stabbed through the back by Jackie Earle Haley in Thomas' dream, causing him to spontaneously die of the same wounds in reality in his jail cell as Andrew Fiscella looks on in horror.
Cinema Verite (2011) [Lance Loud]: Dies (off-screen) of AIDS-related illness, some time after the final scene; his death is mentioned in the on-screen text at the end of the film.
Fear Clinic (2015) [Blake]: Dies after Fiona Dourif physically closes his eyes, allowing him to be released from the grasp of fear.

Terminator: The Sarah Connor Chronicles (2008) [John Connor]: In both "Pilot" and "Some Must Watch, While Some Must Sleep", he's shot to death (by Aaron Cash in the former, Julie Ann Emery in the latter) in Lena Headey's nightmare sequence; he survives the episode in reality.
Star Trek: Picard: Seventeen Seconds (2023) [Titus Rikka]: Gets vaporized by Michael Dorn after reverts to his liquid state.
