Whisper (2007) [Roxanne]: Shot in the chest by Josh Holloway, who had mistaken her for Blake Woodruff; she dies in Josh's arms shortly afterwards. (Thanks to Frank)
The Other Side of the Door (2016) [Maria Harwood]: Stabbed in the stomach with a sickle; she is later seen in purgatory when Jeremy Sisto tries to contact her.
The Show (This Is Your Death) (2017) [Karina Rogers]: Commits suicide by injecting herself with a lethal amount of drugs; she ultimately dies as Josh Duhamel holds her in his arms.

Prison Break: Call Waiting 2007) [Sara Tancredi]: Decapitated (off-screen) by Jodi Lyn O'Keefe. (The character would later be revealed to be alive in the fourth season but this was originally intended as a genuine death scene.)
The Walking Dead: The Killer Within (2012) [Lori Grimes]: Dies of complications of child birth. She is then shot in the head (off-screen) by her son (Chandler Riggs), to prevent her from coming back as a zombie. Her body is later eaten by a zombie.
