- The Hunger Games (2012) [Tribute Boy District 8]: Stabbed to death by Jack Quaid during the initial battle, as they fight to claim weapons and supplies. (I couldn't spot Sam in all the chaos, so I'm not sure whether his death was depicted on-screen or not.) (Thanks to Andrew)
