Jason Goes to Hell: The Final Friday (1993) [Joey B.]: Face bashed in when Steven Culp (who'd been possessed by Jason) hits her in the mouth with his elbow during a struggle in the diner.
Los Locos (Los Locos: Posse Rides Again) (1997) [Sister Drexel]: I vaguely recall that she was killed in this movie, though I can't remember the details.
The Belko Experiment (2017) [Peggy Displasia]: Head explodes after Gregg Henry sets off the explosive implant in her head.

American Dad!: Irregarding Steve (2006) [Mama Squirrel]: Provided the voice of a squirrel. Dies of a heart attack (off-screen) due to obesity. Her body is shown as her sons (voiced by Johnny Galecki and John Krasinski) discover her. She is then later incinerated as the tree is set on fire from lightning.
Heroes: Unexpected (2007) [Dale Smither]: Head sliced open (off-screen) by Zachary Quinto's telekinetic powers in her garage. Her body is (partially) shown afterwards when Sendhil Ramamurthy discovers her.
