The Lightship (1985) [Stump]: Shot by Arliss Howard during the hijacking of a naval lightship.
Total Recall (1990) [Harry]: Throat crushed when Arnold Schwarzenegger uses his foot to break Robert's neck.
Man's Best Friend (1993) [Detective Kovacs]: Mauled to death by Max the genetically enhanced dog.

The Twilight Zone: Healer (1985) [Joseph Rubello]: Presumably dies of untreatable lung cancer shortly after the events in the episode, when Eric Bogosian's healing crystal fails to cure him.
Batman: The Animated Series: Heart of Steel, Part 2 (1992) [Detective Harvey Bullock]: Voicing both the real Bullock and his android duplicate, the android falls into a searchlight while fighting Kevin Conroy on a rooftop. The real Bullock survives the episode.
NYPD Blue: NYPD Lou (1993) [Alphonse Giardella]: Shot by a gunman in his hotel as Sherry Stringfield looks on helplessly.
Murder, She Wrote: Murder in High C (1995) [Rudolfo Petrocelli]: Shot in the chest by John Getz.
Batman Beyond: Rebirth, Part 1 (1999; animated) [Lt. Harvey Bullock]: Although he doesn't appeared in the series or isn't mention, its possible he either died or retired before the events of the series.
Spawn: The Mindkiller (1999, animated) [Mr. Orvetti]: Shot in the back of the head by Eric Roberts as John DiMaggio and Channon Roe watch in horror.

Mafia II (2009) [Joe Barbaro/Derek Pappalardo]: Playing two separate characters, "Derek" is killed by Rick Pasqualone after Rick discovers he is responsible for his father's death and "Joe" is presumably killed after being taken away in a car (his death is suggested in Mafia III, but there is evidence that proves his survival and teases by developers).
