Ben 10: Secret of the Omnitrix (2007; animated) [Lepidopterran Prisoner]: Blown up after being thrown into a security drone by Tetrax (Dave Fennoy).

Power Rangers Zeo: It Came from Angel Grove (1996) [Ay-Gor]: Electrocuted along with David Yost and Nakia Burrise when the experiment goes awry reviving Nakia. The real Alpha 5 survives but is not seen in the episode.
The Grim Adventures of Billy & Mandy: Runaway Pants (2005; animated) [Billy]: In Grim's (voiced by Greg Eagles) imagination, his head explodes after doing a crunch exercise. The plot of this episode was made-up by the "narrator" (voiced by Jim Meskimen), so Grim doesn't really imagine that. (Thanks to Dake).
The Grim Adventures of Billy & Mandy: Modern Primitives (2006; animated) [Billy]: Having the body of a lobster, Richard is boiled alive by Grim in a pot. This is just an imagination that Greg has, so Richard survived the episode. (Thanks to Dake).
Mighty Morphin Power Rangers: Once and Always (2023; special) [Alpha 8]: Transformed into Barbara Goodson's new robotic body when his body is hit by her particles when David Yost tries to restore his falling mentor Zordon. Robo Rita is finally destroyed when Walter Jones shoots her with his Power Axe in Cannon mode.

Helluva Boss: Pilot (2019; animated) [Moxxie/Eddie]: Playing a dual role in the series, "Eddie" is killed after Brandon Rogers shoots him through the chest, then has his body get dismembered by Rogers with the help of "Moxxie" (Played for comic effect.). "Moxxie" survives the series.
