- She Done Him Wrong (1933) [Russian Rita]: Accidentally stabbed with her own dragger during a struggle with Mae West. (Thanks to Constantine)
- Topper Returns (1941) [Lillian the Housekeeper]: Presumably drowned (off-screen) when H.B. Warner activates a trapdoor and drops her into an underground grotto. We only hear her scream after the lights back out. We later learn what happened to her when Eddie 'Rochester' Anderson falls through the trapdoor himself; since Eddie survives, there's a possibility Rafaela did as well, although it's never confirmed.
