- Bad Boys II (2003) [Carlos]: Dies when Gabrielle Union throws her gun on top of trigger for a landmine next to Otto's feet. The landmine then goes off, blowing Otto up.

- Burn Notice: "Long Way Back" (2009) [Diego Garza]: Thrown out of a window by Chris Vance (who makes it look like suicide), his body is later seen when Jeffrey Donovan sees him.
- Person of Interest: Triggerman (2012) [Ochoa]: Presumably killed off camera by Jim Caviezel, the episode ends with Otto being confronted in his hotel room by Jim who's bribery attempt is turned down.
- Gotham: Damned if you Do (2015) [Odgen Barker]: Shot twice in the chest by Ben McKenzie, when Otto attempts to ambush Ben inside a parking garage.
- Luke Cage: Straighten It Out (2018) [Arturo Rey III]: Shot once in the head with a handgun by Theo Rossi, after Otto mocks and threatens Alfre Woodard, while Thomas Q. Jones looks on in shock.
