• Xtro II: The Second Encounter (1991) [Baines]: Killed in an elevator crash, when he deliberately lets the elevator plummet in an attempt to destroy the alien.

• Bad Company (1995) [Jake]: Killed by one of Frank Langella’s assassins (it's left unclear if he died at the end).

• Vertical Limit (2000) [Tom McLaren]: Injected with an air bubble by Bill Paxton, as Robin Tunney looks on from the shadows. (Thanks to ND)

• Ignition (2001) [Peter Scanlon]: Killed by one of Colm Feore’s military assassins (it's left unclear if he died at the end).

• Chaos (2005) [Det. Vincent Durano]: Fatally injured in an explosion from a bomb set off by Jason Statham/Wesley Snipes (it's left unclear how bad his wounds were so I thought i'd list this just in case).

TV Deaths

• The X-Files: Existence (2001) [Alex Krycek]: Shot in the head by Mitch Pileggi when Nicholas orders Mitch to kill David Duchovny. (He returned as a ghost in the subsequent episode The Truth.) (Thanks to ND)
