12 Monkeys (1995) [Teddy]:
Hamlet (2000) [Ophelia]: Drowned in a river. (I haven't seen this version, but we all know the story...)
The Truth About Charlie (2002) [Lola Jansco]: Hit by a car driven by Frederique Meininger when Lisa pushes Thandie Newton out of the way; she dies shortly afterwards as Thandie kneels by her side.
Nine Lives (2005) [Holly]: Possibly commits suicide (off-screen) by shooting herself after confronting her father at his home; the scene ends with her placing the gun to her head, lowering it, placing it in her mouth, and then lowering it again, so it's left up to the viewer's interpretation as to whether or not she goes through with it. (She later appears in one of the other segments, but since the nine stories are not necessarily in chronological order, her reappearance doesn't prove whether or not she dies.) (Thanks to Lauren)
