- The Devil's Rejects (2005) [Adam Banjo]: Stabbed to death (off-screen) after being shot in the neck by Bill Moseley; Bill then skins Lew's head (off-screen) and wears his face as a mask to terrorize Lew's wife (Kate Norby) before placing the mask on Kate.
- The Texas Chainsaw Massacre: The Beginning (2006) [Sheriff Winston Hoyt]: Shot in the head by R. Lee Ermey when Lew tries to arrest Andrew Bryniarski.
- Halloween (2007) [Noel Kluggs]: Head bashed repeatedly against a wall by Tyler Mane. (This scene only appears in the unrated cut of the film; he survives in the theatrical version.)
- The Killing Jar (2010) [Lonnie] Shot to death by Michael Madsen.
- The Lone Ranger (2013) [Hollis]: Shot to death by William Fichtner's men; his body is seen later on when Johnny Depp buries it.
- Wicked Blood (Bad Blood) (2014) [Donny Baker]: Shot by Sean Bean during a fight in Lew's motor home.
- 31 (2016) [Psycho-Head]: Slashed to death by his own chainsaw (having fallen onto it) by Jeff Daniel Phillips.
- Kidnap (2017) [Terrence Fickey]: Killed by Halle Berry.
TV Deaths
- Criminal Minds: Coda (2011) [Bill Thomas]: Shot in the chest by Jessica Lundy on Lew's boat when Lew coaxes Jessica into shooting him. We only hear the shot from outside the boat. His body is shown when Shemar Moore enters the boat and discovers him.
- The Walking Dead: Home (2013) [Axel]: Shot in the head by David Morrissey as he's talking to Melissa McBride; Melissa then uses his body as a human shield in order to survive the shots.
