Suicide Kings (1997) [Elise Chasten]: Shot to death, along with Sean Patrick Flanery, by Denis Leary. (An alternate ending was filmed in which they survive.) (Thanks to Christina)
The Faculty (1998) [Marybeth Louise Hutchinson]: Disintegrated (while in her alien form) when Elijah Wood stabs her in the eye with a pen filled with a dessicating drug (at this point, Laura was replaced with a CGI version of the alien). (Thanks to PortsGuy)

The Outer Limits: Feasibility Study (1997) [Sarah Hayward]: Turned to stone, along with her father (David McCallum) and the other abductees, when they deliberately expose themselves to the alien virus to prevent the aliens from invading Earth. (Thanks to Garrett and PortsGuy)
Dead Like Me: My Room (2003) [Daisy Adair]: Playing one of the undead "reapers," she is introduced in this episode. Died of smoke inhalation during a fire in 1938 (which she claims occurred during the filming of Gone with the Wind); the circumstances of her death have been referred to but never actually depicted. CSI: Fallen Idols (2007) [Diane Kentner]: Commits suicide (off-screen) by taking an overdose of pills; her body is shown afterwards when the investigators discover her beside Matt Lanter's body in her studio.
