Terminator 3: Rise of the Machines (2003) [T-X]: Her human form dies when hit by a helicopter. Playing the endoskeleton form of an advanced android, "she" is destroyed in an explosion after Arnold Schwarzenegger places a bomb in her "mouth".
Curse of the Ring (2004) [Brunhild]: Commits suicide by stabbing herself on the funeral pyre of her lover.
S.W.A.T: Firefight (2011) [Rose Walker]: Commits suicide by shooting herself in the head.
Bounty Killer (2013) [Catherine]: Shot in her lower chest with a shotgun by Matthew Marsden, which sent her sliding on the floor on her back. He then took a shot at the company metal insignia hanging over her, which fell over her body, decapitating her.
Hunting the Phantom (2014) [Rush]: Shredded off-camera by the ships propeller after Sanjar Madi pushed her towards it during an underwater fight.

Mortal Kombat: Conquest: Vengeance (1999) [Taja]: Killed off-screen while in a hand-to-hand fight with Shadow Priests alongside Daniel Bernhardt and Paolo Montalban. (Note: If the show hadn't cancelled, her character like everyone would've been brought back to life.)
Painkiller Jane: Pilot (2007) [Jane Vasco]: Killed in a fall, we see her body as police investigate and again when she comes back to life inside her body bag.
Painkiller Jane: Ghost in the Machine (2007) [Jane Vasco] Hit by a train. We see her body lying still under her regenerative powers cause her to come back to life.
