Monument Ave. (1998) [Mickey Pat]: Killed in a fight/shootout with mobsters.
In Too Deep (1999) [O'Hanlon]: Shot repeatedly in the stomach and chest by police sharpshooters after Kevin bursts out of a door with his gun drawn.
The Boondock Saints (1999) [Chappy] Shot in the chest by Willem Dafoe while Willem is disguised as a woman.
Hard Luck (2006) [Franklin]: Shot repeatedly in the head and neck by Wesley Snipes after Kevin wounds Jacquelyn Quinones.
Unknown (2006) [Detective McGahey]: Killed by one of the kidnappers (i'm not sure if he died or not).
Black Dynamite (2009) [O'Leary] Shot in the stomach by Michael Jai White. (Played for comic effect)
Assassination Games (2011) [Culley]: Tortured to death (off camera) by Ivan Kaye and his cohorts (after having his ear cut off).
Exeter (2015) [Greer]: Neck snapped by the possessed Michael Ormsby.

Sons of Anarchy: Fun Town (2008) [Michael McKeavey]: Killed in a struggle with Lyle Kanouse’s cohorts (it’s revealed he’s been killed in between episodes).
Madso's War (2010 TV Movie) [Gerry Walker]: Presumably dies of blood loss (off-screen) after being shot in the side by Timothy V. Murphy with a shotgun.
The Punisher: The Dark Hearts of Men (2017): [Kusack]: Beaten to death by Josh Stewart, after having attempted to have Josh killed in an ambush. Josh is able to break Kevin's fingers, before throwing him to the ground, and punching his skull repeatedly while wearing knuckle dusters, leaving Kevin's face beyond recognition.
