Mackenna's Gold (1969) [Hesh-Ke]: Falls off the edge of a mountainside trail while trying to force Camilla Sparv over the edge.
The Maltese Bippy (1969) [Carlotta Ravenswood]: Shot in the chest during a big stand-off. (Played for comic effect.) (Thanks to Energman and HH)

Batman: Better Luck Next Time (1966) [Catwoman]: Loses grip and falls into the bottomless chasm when Adam West tries to save her. (She of course, returned in future episodes, because of her "nine lives" ability, but I will list it just in case).
Batman: Scat Darn, Catwoman (1967) [Catwoman]: Falls off a building ledge when Adam West and Burt Ward try to capture her. (like previous, she of course, returned in future episodes, because of her "nine lives" ability, but I will list it just in case)
Columbo: Double Shock (1973) [Lisa Chambers]: Pushed off of a balcony (off-screen) by Martin Landau. Her body is seen lying on the pavement when Tim O'Connor sees it. (Thanks to Garrett)
Buck Rogers in the 25th Century: Flight of the War Witch Part 2 (1980) [Zarina]: Killed in an explosion when a portal shuts down while her ship is halfway through it. (Thanks to PortsGuy)
Batman: The Brave and the Bold; Chill of the Night (2010; animated) [Martha Wayne]: Shot to death, along with her husband Thomas (voiced by Adam West) by Joe Chill (voiced by Peter Onotari) in an alleyway while their son Bruce (voiced by Zachary Gordon) looks in in horror. Her death is shown in flashback as the older version of Bruce (voiced by Diedrich Bader) remembers their deaths.
