Nixon (1995) [Pat Nixon]: Dies (off-screen) of emphysema on June 22, 1993. Her death is mentioned in the end credits.
When the Sky Falls (2000) [Sinead Hamilton]: Shot to death (off-camera) by a gunman on a motorcycle while her car is stopped at a red light; the scene cuts to an overhead view of the car as he fires repeatedly, and her body is not shown afterwards. (See also Cate Blanchett in Veronica Guerin both are based on the same true story, though this version changes the names.) (Thanks to Gary)
The Notebook (2004) [Anne Hamilton]: Dies (off-screen), presumably of natural causes, at some unspecified time between the 1940s-era flashbacks and the present-day scenes.
Death Race (2008) [Warden Claire Hennessey]: Killed in an explosion, along with Jason Clarke, when Ian McShane sends her a bomb disguised as a present for setting a viewership record for the Death Race. (Thanks to Bobby)

The Mists of Avalon (2001) [Morgause]: Stabbed in the stomach with her own dagger during a struggle with Anjelica Huston, after Joan attacks Anjelica for denouncing her. (Thanks to Milou).
