Mission to Mars (2000) [Renee Cote]: Hit in the face by a large rock when a windstorm on the Martian surface sends it crashing through the faceplate of her spacesuit.
Along Came a Spider (2001) [Officer Tracie Fisher]: Falls to her death after the car she's riding in crashes over the edge of a dam, then falls despite Morgan Freeman and the other cops' efforts to pull the car up to safety. We see a close-up of her screaming as the fall begins, then a long shot of the car falling into the distance.

Highlander: The Series: Reluctant Heroes (1995) [FBI Agent Kayla Brooks]: Shot to death by Peter Outerbridge while kissing her, after Peter double-crosses her.
The Outer Limits: Criminal Nature (1998) [Carolyn]: Mauled/slashed to death (off-screen) by a mutant in an elevator. Her body is shown afterwards when Gary Cole arrives at the scene.
Independence Day-saster (2013 TV) [Spears]: Crushed (along with Michael Kopsa) when the underground base collapses when the aliens attack it.
Van Helsing: The Beholder (2019) [President Archer]: Bitten on the neck and drained of blood by Tricia Helfer (in disguise as Nicole Muñoz), and then Tricia assumed her identity and her physical form.
