Kill Bill: Vol. 1 (2003) [Mrs. Harmony]: Though she doesn't appear alive in this film, she is shot to death (off-screen) by the Deadly Viper Assassination Squad, along with everyone else in the chapel - save for Uma Thurman - on the orders of David Carradine; the sequel, Kill Bill: Vol. 2, shows her character alive in a flashback and we hear the massacre from outside the chapel.
Quarantine (2008) [Mrs. Espinoza]: Bludgeoned to death with a hammer by Jay Hernandez - on top of having been previously infected with the rabies-like virus and shot in the chest by Columbus Short - after she suddenly reappears to attack Jay. Tales from the Crypt: Television Terror (1990): [Ada Ritter]: Committed suicide through unknown means years prior to the episode. Her ghost appears to terrorize Morton Downey Jr..
Monk: Mr. Monk and Little Monk (2005) [Gladys Aquino]: Hits her head on an end table when Brent King and Michael Dunn toss her around during a struggle, after Brent and Michael break into Donna Bullock's house at the request of David Hunt (who thought the house would be empty).
