Cherry Falls (2000) [Leonard Marliston]: Shot in the chest by Amanda Anka, after Jay suddenly jumps up and attacks after being knocked off a roof and impaled on a fence post at the end of a struggle with Brittany Murphy. (Thanks to Gary, Jarryd and Keith)
Even Money (2006) [Augie]: Poisoned (off-screen) by Tim Roth. We learn of his death afterwards when Carla Gugino informs Grant Sullivan.

Saturday Night Live: David Duchovny/Rod Stewart (May 13, 1995) [Jay Mohr]: In the "Polar Bear Sketch", he is mauled to death (off-screen) after jumping in the pit. His screams are heard and his blood gushes over Tim Meadows, Chris Farley, Norm MacDonald, and Adam Sandler.
Action: Lights, Camera, Action (1999) [Peter Dragon]: Dies of a heart attack. (Thanks to Tommy)
Law & Order: Criminal Intent: Rispetto (2011) [Nyle Brite]: Shot to death (off-screen) by Neal Huff; his body is shown afterwards when Vincent D'Onofrio and Kathryn Erbe discover him.

Saints Row 2 (2008) [Dane Vogel]: Is shot in the head by the Protagonist, and Jay's corpse falls several stories while hitting multiple surface (aftermath played for comedic effect)
Saints Row: Gat out of Hell (2015) [Dane Vogel]: Due to Jay's death in Saints Row 2, Jay is a denizen of the Hell Dimension, serving as Daniel Dae Kim's guide in the game.
