Angel of Death (2009) [Cameron Downes]: Strangled by Zoë Bell with a ziptie while slashing Zoë with a straight razor.
Percy Jackson: Sea of Monsters (2013) [Luke Castellan]: Possibly eaten by Robert Maillet/Ron Perlman (off-screen); we last see Robert/Ron preparing to eat him. (The character was presumably intended to survive, but because future films were cancelled, his fate is left unclear.)
An Affair to Die For (2019) [Everett Alan]: Stabbed in the stomach by Claire Forlani; he dies after speaking with Claire.
Malignant (2021) [Derek Mitchell]: Head ripped off by Ray Chase, when Annabelle Wallis is having visions.

Supernatural: Jump the Shark (2009) [Adam Milligan / Ghoul]: "Adam Milligan" is killed by "Ghoul" prior to the events of the episode; his body is shown when his half-brothers (Jared Padalecki and Jensen Ackles) find him and his identity is assumed by "Ghoul". He is later brought back to life in the episode Point of No Return. "Ghoul" has his head bashed in by Jensen.
Another Life (2019) [Sasha Harrison]: Shot to death by A.J. Rivera while strangling JayR Tinaco.
Supernatural: Inherit the Earth (2020) [Adam Milligan / Michael]: Playing a human and angel sharing possession of the same body, "Adam Milligan", the human, is killed by Rob Benedict with his powers (off-screen); his death is revealed when "Michael", the angel, informs his half-brothers (Jared Padalecki and Jensen Ackles) and Alexander Calvert and is presumably undone by Alexander with his powers. "Michael" is killed by Rob with his powers.
