The Plague of the Zombies (The Zombies) (1966) [Alice Tompson]: Killed (off-screen) by a voodoo spell. Her body is shown afterwards when Diane Clare sees her being carried by a zombie, and then when her body is found by the police, and yet again when Andre Morell and Brook Williams examine her. She later comes back to life as a zombie, only to be decapitated by Andre Morell. (Thanks to Ezeckial)
The Reptile (1966) [Anna Franklyn]: Burned to death (off-screen) after being trapped in a burning house while in the reptilian form. We see her lying unconscious inside the house before Ray Barrett and Jennifer Daniel escape, followed by an exterior shot of the burning building.

Callan: Once a Big Man, Always a Big Man (1969) [Eva]: Reportedly shot to death, although my source couldn't remember for certain. (I haven't seen this episode myself and since the master tape was apparently destroyed during the BBC's "house cleaning" it will probably never be seen again). (Thanks to Waite)
Special Branch: Catherine the Great (1974) [Helga]: Shot to death. (Thanks to Waite)
Doctor Who: The Two Doctors (1985) [Chessene]: Killed by the energy backlash after attempting to escape in a malfunctioning time machine. After death, her body reverts from her human form to her original alien appearance.
