Too Late the Hero (Suicide Run) (1970) [Signalman Scott]: Shot in the head by a Japanese soldiers while using the Japanese radio.
The Lost World: Jurassic Park (1997) [Ajay Sidhu]: Ripped to pieces off-screen by the Velociraptors and he missing when Vince Vaughn finds his backpack throw in the grass. His death is reported later when Pete Postlethwaite is talking to Arliss Howard. (Thanks to Neil)

Hawaii Five-O: Ring of Life (1975) [Ram Bushan]: Shot by Don Knight after being double-crossed by Knight and Penelope Windust.
Captains and the Kings (1976 TV mini-series) [Haroun 'Harry' Zieff]: Drowned (off-screen) in the sinking of the Titanic; we see him standing on the ship and waving goodbye to the lifeboats in a black & white flashback when a survivor tells Richard Jordan how Harvey saved her.
Airwolf: Mind of the Machine (1984) [Alexi Provov]: Gets blown out of the sky along with Sondra Currie by Airwolf.
Star Trek: The Next Generation: The Big Goodbye (1988) [Felix Leech]: Playing a virtual-reality Holodeck creation, he ceases to exist (along with Laurence Tierney) when they leave the Holedeck in an attempt to take over the real world. (Thanks to Stephen and Neil)
