The Factory (2012) [Darryl]: He dies (off-screen) of a drug overdose in his hotel room. We see him lying in his bed when Jennifer Carpenter and John Cusack arrive to arrest him.

Star Wars: The Clone Wars: Prisoners (2011) [Riff Tamson]: Killed when Lee-Char (Adam McArthur) fires an explosive knife in his chest and he explodes.
Mad: S Cape From Planet Earth/The X-Mentalist (2013; animated) [Steve Harvey/Lex Luthor/Suspect]: Voicing three characters, Lex Luthor is punched down a number of stories by Superman (Thomas F. Wilson) (played for comic effect).
Rick and Morty: Anatomy Park (2013) [Poncho]: Fell off a balcony to his death.

Harley Quinn; Another Sharkley Adventure (2022; animated) [Brother Prince Shark]: Stabbed in the stomach by his brother Ron Funches with Alan Tudyk's sharp mask.

Mass Effect (2007) [Elanos Haliat/Charles Saracino]: "Haliat" is shot to death by during gameplay Mark Meer/Jennifer Hale or one of their squadmates; "Charles" survives the game.

Alpha Protocol (2010) [Alpha Protocol Director Yancy Westridge]: Depending on player choice, the player can engage Gary in a boss battle if the player chose to ally with Fred Tatasciore. Shortly after defeating Gary, the player has the choice to arrest Gary or execute him via gunshot wound to the head,
StarCraft 2: Heart of the Swarm (2013) [Dominion General Horace Warfield]: Impaled by building beams while in battle with Tricia Helfer’s Zerg forces. Tricia herself uses her psychic powers to drive the beams further into him when Gary taunts her about Robert Clotworthy.
