Dark House (2009) [Paula]: Neck broken when she falls down the stairs while running away from the holograms made "real" by Diane Salinger's spirit.
Cold Comes the Night (2013) [Amber]: Shot in the head by Bryan Cranston as she goes to attack Alice Eve

Cold Case: Pin Up Girl (2008) [Rita Flynn]: Shot in her chest by Chelsea Hobbs. Her body is initially seen at the start of the episode. Her death is later seen in a flashback. She later appears as a ghost/figment of imagination to Harold Gould, who flashes back his younger self (Patrick Fischler), at the end of the episode when her murder is solved.
Spartacus: Old Wounds (2010) [Sura]: Dies in Andy Whitfield's arms from wounds sustained in a "bandit attack", actually carried out by Mark Mitchinson on John Hannah's orders, in a plan to break Andy's will and keep him in check.
The Flash: Blocked (2018) [Vanessa Jansen/Block]: Impaled through the back of the chest with a lightning-shaped dagger by Chris Klein as Jessica Parker Kennedy looks on shock. She was later killed off-screen by him.
NCIS: Hail & Farewell (2019) [Major Ellen Wallace]: Murdered off-screen by Doug Savant via head trauma in order to cover up Doug’s role in committing war crimes in Kosovo. Erin’s skeletal remains are found in the beginning of the episode by J. Teddy Garces.
