The Rage: Carrie 2 (1999) [Mark Bing]: Drowns in the pool as the cover closes after being pulled in by Emily Bergl, then hitting his head on a pool cover bar, when she uses her powers to turn said cover on.
The One (2001) [Yates]: In an extended cut of the film, Dylan and Richard Steinzmentz attempt to enter an exploding power plant in order to apprehend Jet Li. Considering that both versions of Jet Li and Jason Statham are evacuated via wormhole during the time of the plant's destruction, it could be assumed that Dylan and Richard have perished.

NCIS: Rule Fifty One (2010) [Jason Paul Dean]: Shot repeatedly in the chest (off-camera) by Michael Weatherly in a shoot-out between Michael, Cote de Pablo, Sean Murray and his henchmen. We only see Michael firing. His body is shown later on in the morgue.

The Mentalist: Red Gold (2011) [Dean Puttock]: Shot to death by Aunjanue Ellis as a suicide by cop while using Simon Baker as a human shield.
Narcos: The Palace in Flames (2015) [Barry Seal aka McPickle]: Machine gunned to death by Wagner Moura men.
Hawaii Five-0: Waimaka 'ele'ele (2017) [Lee Campbell] Impaled on a piece of metal rebar at the end of a fight with Alex O'Loughlin
