- Hellbound: Hellraiser II (1988) [Pinhead a.k.a Lead Cenobite/Capt. Elliot Spencer]: Throat slashed with a tentacle by Kenneth Cranham, after he and the other Cenobites use chains to restrain him, to give Ashley Laurence and Imogen Boorman time to run, as Doug reverts to his human form in front of them. (the character is brought back in the 1992 sequel Hellraiser III: Hell on Earth).
- Nightbreed (1990) [Dirk Lylesberg]: Shot in the head by one Charles Haid's lynch mob (causing him to get a chain wrapped round his throat) as he is freeing the night-breed berserkers, he dies as Craig Sheffer kneels by his side (Doug was wearing heavy monster makeup for this role).
- Proteus (1995) [Leonard Brinkstone]: Mauled to death or devoured (along with several of his bodyguards) by the Proteus mutant while it's disguised as Nigel Pegram (Doug was wearing disfigured makeup for this role).
- Hellraiser IV: Bloodline (1996) [Pinhead]: Vaporized having been crucified when Bruce Ramsay traps him in a giant artificial Lament Configuration disguised as a space station which then explodes (since this takes place centuries in the future Doug Bradley would later reprise his role as Pinhead in other films which take place before the events of this film).
- The Prophecy: Uprising (2005) [Laurel]: Shot in the head by Sean Pertwee while he was about to attack Sean.
- The Exorcists (2023) [Father Patrick Ryland]: Dies of a heart attack brought on by the events of the film after allowing the demon Lamech (voiced by Michelle Bauer) to take him as a host in the middle of the heart attack, therefore taking the demon with him.
