Scary Movie 4 (2006) [Fight Referee]: Neck accidentally snapped when he falls against the ropes in the boxing ring, in a parody of Hilary Swank's paralyzing injury in Million Dollar Baby. (Played for comic effect)

21 Jump Street: You Oughta Be in Prison (1987) [Vincent Montgomery]: Killed in an accident along with R.G. Miller on a prison bus while transporting Reginald T. Dorsey.
Highlander: The Series: Bless the Child (1994) [Luke Hoskins]: Shot in the chest by his brother (Johnny Cuthbert) in the woods.
Titanic (1996 TV) [Bartender Thomas Joe McCormack]: Drowns or freezes to death (off-screen) during the sinking of the RMS Titanic. (Historically inaccurate, there was no barman in the First Class Smoking and the real Thomas Joseph McCormack was a 19 year old Third Class survivor and later died on November 4th, 1975).
Supernatural: Ask Jeeves (2014) [Detective Howard]: Killed (off-screen) by Izabella Miko. His body is later found by Jensen Ackles and Jared Padalecki in his hotel room's bathroom, his head in the toilet.

The Godfather (2006) [Don Vito Corleone/Capt. McCluskey]: As Capt. McCluskey, Doug is shot in the throat by Joseph May during a meeting in a diner, who then shoots him in the head to finish him off. As Don Vito Corleone, he dies of a heart attack off screen. See also Sterling Hayden and Marlon Brando's deaths in The Godfather.
