Batman Returns (1992) [Penguin's Mother]: Dies (off-screen) under unspecified circumstances, at some point between the opening prologue and the main part of the film; her death is confirmed when Danny DeVito visits her grave.
Dark House (2009) [Janet Darrode]: Commits suicide by mangling her hands in a garbage disposal, as Courtney Robinson looks on in horror. She returns as a ghost years later when Meghan Ory (Courtney's character as an adult) returns to the house. (It's suggested towards the end that Diane's ghost was only in Meghan's mind, but the final scene again suggests that the ghost is real.)
Slay Belles (2018) [Cherry]: Explodes after Barry Bostwick, who she has a supernatural connection to, is decapitated by Kristina Klebe.

Star Trek: Deep Space Nine: The Darkness and the Light (1997) [Lupaza]: Killed (off-screen) along with William Lucking when Randy Oglesby remotely attaches a bomb to the window of their room on the space station causing them to be blown out into space.
Charmed: A Witch's Tail: Part 1 (2002) [Sea Hag]: Life force Sucked up when a Mermaid Alyssa Milano threw a Auger Shell on her, causing her to turn into Dust.
Power Rangers Lightspeed Rescue: The Fate of Lightspeed: Part 2 (2000) [Queen Bansheera]: Dragged into the Shadow World by Neil Kaplan and other demons just as the Skull Cavern collapses and turns to dust.
Salem: The Red Rose and the Briar (2014) [Rose Browning]: Decapitated by Elise Eberle, as Janet Montgomery watches on.
