Don't Blink (2014) [Noah]: Disappears from existence when he's dragged outside by Zack Ward after Zack shoots him, believing him to have something to do with other characters disappearing. We last hear him screaming from behind a closed door before the screaming suddenly stops.

Legend of the Seeker: Hartland (2009) [Michael Cypher]: Impaled by a D'Haran during a battle; dies shortly afterwards while talking to Craig Horner.
Beauty and the Beast: Liar Liar (2013) [Sebastian Clifton]: Falls to his death from top of an elevator by Vincent Keller during a battle with Kristin Kreuk. He is last seen as Vincent jumps down and checks on his body. I'm not sure if this counts or not but I'll have it here just in case since he is not seen again afterwards.
