Disturbing Behavior (1998) [Tom Cox, Blue Ribbon]: Falls to his death (along with all the other mind-altered "Blue Ribbon" students) when William Sadler drives his truck over the dam after all the Blue Ribbons climb on board to get at William's sonic devices. (I couldn't spot David specifically among all the students, but it's stated that all of them are there.)

Final Destination 2 (2003) [Evan Lewis]: Impaled through the head when a fire-escape ladder falls on him, after he narrowly escapes a fire in his apartment. Later, his body is shown being disposed of by incineration at Tony Todd's mortuary. (He also burns to death after being trapped in his car in A.J. Cook's premonition of the future, but she managed to prevent that from happening.) (Thanks to Corey, Velvett, Gordon, Callum, Sean, and Francisco)

I'll Always Know What You Did Last Summer (2006) [Colby Patterson]: Stabbed in the mouth with a hook by Don Shanks; his body is shown again later on when Ben Easter and Brooke Nevin discover him.

Aliens vs Predator: Requiem (2007) [Dale Collins]: Burned to death by the alien's acidic blood when the Predator (Ian Whyte) shoots the alien, splattering its blood on David's face. (Thanks to Andrew)

A Mother Betrayed (2015 TV) [Jonathan]: Fatally wounded in a car crash, with his wife Lynn Collins tearfully holding him as he dies; he briefly appears later in the movie as either a spirit or a figment of Lynn's mind.

Supernatural: Two and a half men (2010) [Mark Campbell]: Neck snapped by the alpha shapeshifter.
