Witchery (La Casa 4; Ghosthouse 2; Witchcraft) (1988) [Gary]: Stabbed in the back by a supernatural entity.
Jekyll & Hyde: The Musical (2001) [Dr. Henry Jekyll/Mr. Edward Hyde]: Impales himself onto George Merritt's sword, dying as Andrea Rivette comforts him.

Knight Rider: Goliath Returns (1984) [Garthe Knight/Michael Knight]: Playing dual roles as Garthe and Michael Knight. As Garthe Knight, he was killed alongside with Ann Turkel after the truck Goliath driven by him crashes into the sea. Michael Knight lives throughout the whole series.
Avalanche (1994 TV) [Duncan Snyder]: Crushed to death/buried alive by the snow when Michael Gross and his children drive into one of the cabin's support columns.
Nightman: Pilot Part 2 (1997) [Katrina's Associate]: Thrown out a window by Matt McColm and falls down a cliff.
Nick Fury: Agent of Shield (1998 TV) [Nick Fury/Nick Fury Robotic Double]: Playing dual roles as Nick Fury and one of his robotic doubles. The Nick Fury double is shot to death by Sandra Hess. Nick Fury survives the rest of the film.
Anaconda: The Offspring (2008 TV) [Hammett]: Killed in an explosion along with the snake after Crystal Allen sets up an explosive after getting stabbed in the stomach in a fight with Crystal.
Sharknado 5: Global Swarming (2017) [Col. Gilbert Shepard]: David didn't appear in the film, but it's mentioned by his grandson (Cody Linley) that he was killed by sharks. (The backstory of The Last Sharknado: It’s About Time he was brought back to life in the new timeline.)
