  • Mass Effect 2 (2010) [Jack/Lilith]: Like the other squadmates, "Jack" can be killed in various ways during the final mission like dying in a ship accident during the approach to the Collector base, dragged away by seeker swarms if the wrong biotic is chosen, shot by the Collectors while leading a fire team or being tasked with holding the line, killed off-screen by the Collectors while escorting the Normandy crew, or crushed to death by rubble during the final boss battle; "Lilith" is liquefied by the Collectors after being captured by them.
  • Mass Effect 3 (2012) [Jack]: Killed in combat by Mark Meer/Jennifer Hale and their squad if the Grissom Academy mission isn't done in time, and she's captured and turned into a Phantom by Cerberus; she can also die in two of the game's endings; (Destroy) she dies, alongside the rest of the surviving cast, when Mark/Jennifer destroys the Reapers, if not enough war assets have been acquired; (Refusal) she is harvested, alongside all sentient life in the galaxy, by the Reapers.
  • Fallout 4 (2015) [Nora/Sole Survivor]: If the player chooses Brian T. Delaney as their playable character, "Nora" is shot in the head by Keythe Farley during a struggle with him while the latter attempted to take her son. If the player chooses Courtenay, Brian dies in her stead, but she can also permanently die if the player does not save at all and dies during gameplay. However, in the endings, Courtenay survives as the player.
