Bundy: An American Icon (2008 Video) [Ted Bundy]: Executed in the electric chair.
Robocroc (2013) [Jim Duffy]: Bitten by robot crocodile.

Parker Lewis Can't Loose: Write or Die (1993) [Parker Lewis]: Machine-gunned (off-screen) along with Billy Jayne and Troy W. Slaten by Maia Brewton and Jennifer Guthrie in a fictional story they are writing at school. The scene turns to black before the shooting, we only hear the firing.
The Stand (1994 TV miniseries) [Harold Lauder]: Commits suicide (off-camera) by shooting himself in the head after being badly injured in a motorcycle crash caused by Jamey Sheridan; we hear the shot as Gary Sinise senses Corin's death from miles away.
Smallville: Zero (2002) [Jude Royce/Double]: Shot in the chest by Judy Taylor as he was attacking Michael Rosenbaum. It was at first thought that either Michael or Michasha Armstrong had shot him during a struggle with a gun or if he survived, but the truth was eventually revealed. The double gets shot to death by Eric Breker while he was torturing Michael and came and supplied his own brand of torture.
House of Bones (2009) [Quentin French]: Imapled with a branch thrown by Ricky Wayne who was possessed by demonic spirits.
