Mary Shelley's Frankenstein (1994) [Victor's Mother]: Died of childbirth (off-screen); her body is shown afterwards when her so, Kenneth Branagh, comes into the room.

Tis Pity She's a Whore (1980 TV movie) [Annabella]: Stabbed to death by her brother (Kenneth Cranham), who then cuts her heart out. Cherie was pregnant at the time of her death, consequently, her unborn child also died. (See also Charlotte Rampling in the 1971 version.)
Oliver Twist (1982 TV movie) [Nancy]: Presumably beaten to death by Tim Curry. (I haven't seen this version but I'm familiar with the story).
Praying Mantis (1983 TV movie) [Beatrice Manceau]: Hit by a truck when she runs across the road in panic, we only see her screaming just before the impact. (Thanks to Cryptic)
Midsomer Murders: The Green Man (2003) [Lillian Webster]: Shot in the head by Jamie Thomas King.
