YellowBrickRoad (2010) [Erin Luger]: Jaw torn off halfway and left leg cut/torn off by Clark Freeman. Her friends later discover her body dressed and posed as a scarecrow.
Fender Bender (2016) [Jennifer]: Stabbed multiple times in the back by Bill Sage.

Cold Case: True Calling (2008) [Laura McKinney]: Shot by David Julian Hirsh while running from him, while Jon Budinoff looks on, helplessly. Her body is shown at the beginning of the episode, and her death is shown in a flashback later on. She later appears as a ghost/figment of the imagination to Jon's older self (Andres Hudson), who flashes back to his younger self, at the end of the episode when her murder is solved.
Smallville: Pandora (2009) [Tess Mercer]: In a vision of the future, she is shot in the stomach with a crossbow bolt by Allison Mack; she dies in Justin Hartley's arms shortly afterwards. She survives the episode in reality. (Thanks to Rowdy)

The Vampire Diaries: The Murder of One (2012) [Sage]: Dies due to her supernatural connection to Casper Zafer after Casper is staked through the heart by Zach Roerig. (Thanks to Cody)
Once Upon a Time: Tiny (2013) [Jack]: Stabbed with her own poisoned sword in her stomach off-camera by a giant (Abraham Benrubi), after she stabs him in the foot several times. She dies shortly after Josh Dallas leaves her behind. (Thanks to Frank)
