Smokin' Aces 2: Assassin's Ball (2010) [Kaitlyn 'AK-47' Tremor]: Shot in the chest first and then head by Clayne Crawford after she opens fire on the FBI agents.
So Undercover (2012) [Bizzy]: Shot to death (off-screen) by a hitman working for Jeremy Piven; we learn of her death afterwards when Jeremy informs Miley Cyrus (and shows her a newspaper article on her death). (Thanks to ND)
Sully (2016) [Tess Sosa]: Incinerated when the plane Tom Hanks and Aaron Eckhart are piloting crashes into a building as they attempt to return to LaGuardia Airport (The crash was only Hanks' nightmare and she survives the movie in reality).

CSI: Invisible Evidence (2003) [Rachel Lyford]: Stabbed in her chest through her collarbone area with a knife by James Marshall after raping her. Her death is shown in flashback.
Cold Case: The Boy in the Box (2004) [Sister Grace Ashley]: Dies of old age/natural causes, shortly before the episode begins. She's seen as a young woman in flashbacks to 1958. She later appears as a ghost / figment of imagination, along with Lyndon Smith at the end of the episode.
Pushing Daisies: Bzzzzzzzzz! (2008) [Kentucky Fitz]: Stung to death (off-screen) by bees; her body is shown in the morgue when Lee Pace brings her back to life by touching her. She dies once again when Lee touches her again.
