Friday the 13th (2009) [Lawrence]: Hit in the back with an axe when Derek Mears throws it at him, then impaled when Derek pushes him back onto the blade.
Final Destination 5 (2011) [Nathan Sears]: Crushed to death when a piece of flaming debris from the plane crash falls through the roof and lands on him. (Additionally, in Nicholas D'Agosto's earlier vision of the collapsing bridge, he is knocked off the bridge by a falling support cable.)

Cold Case: Colors (2005) [Clyde Taylor]: Beaten to death with his own baseball bat by Eric Ladin; shown in a flashback when Eric's older self (Thomas Kopache) confesses. Arlen appears as a spirit or vision when Thom Barry watches his nephew's (Kameron Winston-Lucas) baseball game.
NCIS: The Enemy Within (2015) [Randall White]: Commits suicide by shooting himself in the head with a homemade gun during a standoff with Mark Harmon, Michael Weatherly and Emily Wickersham while Mark, Michael and Emily look on in shock.
