The Butterfly Effect (2004) [Kayleigh Miller]: Commits suicide (off-screen, exact method unspecified); we learn of her death afterwards when William Lee Scott angrily informs Ashton Kutcher over the phone. Ashton later projects his consciousness back in time to prevent her death. (Thanks to Nadine)

Mirrors (2008) [Angela Carson]: Jaw torn open when her reflection (controlled by a trapped demon) in the bathroom mirror tears its own mouth apart. Her body is later seen as her brother (Kiefer Sutherland) returns to her home and finds police investigate, then again as he kneels by her side holding her hand. Flight 7500 (2014) [Pia Martin]: Deprived of oxygen after the plane she's traveling on suffers uncontrolled decompression. She remains on the plane unaware of her death until she sees her body and later "dies" again when she allows her soul to be collected by a spirit.
Patient Seven (2016) (Mom) (segment: "The Visitant"): Killed off-screen, we know she's dead because her murder is the cause of Anna Rose Moore being locked up in the asylum. (Spoiler alert) But it all turned out to be Michael Ironside's dream.
