Jason Goes to Hell: The Final Friday (1993) [Vicki Sanders]: Impaled through the stomach and out the back with a skewer by Steven Culp, who had been possessed by Jason (Kane Hodder), when she shoots at him several times to help John D Le May and Keri Keegen escape. The uncut version extends this scene a bit, where Steven also crushes Allison’s head with his bare hands.
Holes (2003) [Linda Miller Walker]: Dies sometime before the events of the film; she only appears in flashbacks.

Deadly Games: The Divorce Lawyer (1995) [Jill Stewart]: Electrocuted/incinerated into ashes (off-camera) by Victoria Rowell in a restroom; we only see flashes of light and smoke from underneath the restroom door.
House, MD: The Mistake (2005) [Kayla McGinley]: Dies (off-screen) of liver cancer, before the episode begins; she appears in flashbacks throughout the episode, interspersed with the inquiry into Jesse Spencer's handling of the case.
Ghost Whisperer: The One (2006) [Lisa]: Killed (off-screen) when a plane crashes into her car; her body is shown and she appears as a ghost afterwards. (Thanks to Lee)
