Heart and Souls (1993) [Penny Washington]: Killed in a bus crash (along with Charles Grodin, David Paymer, Kyra Sedgwick and Tom Sizemore); their spirits appear to Robert Downey Jr. (occasionally taking over Robert's body) throughout the rest of the movie.
Crooklyn (1994) [Carolyn Carmichael]: Dies (off-screen) of cancer; we last see her in the hospital, and her death is confirmed in a subsequent scene when her daughter Zelda Harris mentions her funeral.
Holiday Heart (2000) [Wanda]: Hit by a car driven by a drug dealer, as Ving Rhames looks on helplessly.
The Forgotten (2004) [Anne Pope]: Sucked into the sky by the aliens when she comes close to learning their secret. Whether or not she actually died is unknown.
Annabelle (2014) [Evelyn]: Sacrifices herself by falling from a window in order to save Annabelle Wallis and Ward Horton from a demon.
The Gray Man (2022) [Margaret Cahill]: Blows herself up after a group of guards raid her building after saving Ryan Gosling.
Salem's Lot (2024) [Dr. Cody]: Shot with a shotgun by Debra Christofferson after she opens a car trunk containing Debra's vampire daughter Makenzie Leigh.

Static Shock: Flashback (2003; animated) [Jean Hawkin]: She mention by her son: Virgil (Phil LaMarr) that she was shot by a stray bullet. When Static, Gears (Jason Marsden), Timezone (Rachel MacFarlane), and Ebon (Gary Sturgis), Virgil attempts to save her, but couldn't. She is however mentioned throughout the series and only appears in the past day segment.
The Last Ship: Unreal City (2014) [Acting President Amy Granderson]: Injects herself in the neck with a lethal syringe after refusing to stand trial for her various crimes as president and as Eric Dane and Woodard's guards look on in shock.
A Series of Unfortunate Events: The Wide Window Part 2 (2017) [Aunt Josephine]: Devoured by leeches after being thrown overboard a ship by Neil Patrick Harris.
Luke Cage: They Reminisce Over You (2018) [Mariah Dillard]: Dies after being kissed with Gabrielle Dennis’s poisoned lipstick. Alfre spits out blood and collapses while speaking to Mike Colter, telling him that the poisoned burned, before she dies in Mike's arms.
